Your assessment is good for 45 days.

Exception: If you were in custody and remained in custody for 45 days after our assessment, it would still be valid.

We are in-network with UCare, HealthPartners, Blue Cross Blue Shield of MN, Hennepin Health, South Country Health Alliance, Direct Access, Medical Assistance Tricare and UnitedHealthcare (UBH/Medica). 

If you need a medical cab to transport you to and from treatment, we can arrange for this; however, medical transportation is a service provided through your insurance company.
